A care plan, the Honu way.

This is what members receive: their family's steps, in order, with the time each one takes. This sample is for an example family. Yours will be built for your family based on the ten answers you provide us in Step 1.

The example family: Margaret, 82, lives alone in her own home. Her daughter noticed two falls this summer, meals thinning out to toast, and mail piling up. Two siblings live out of state. Nothing has been decided, and nobody has talked about it yet.
